About the treatment

How it works

The tear trough is the groove that runs from the inner corner of the eye across the top of the cheek. When it deepens it casts a shadow, which is often what people mean by dark circles.

A small amount of soft hyaluronic acid filler placed deep in the groove can smooth the step between eyelid and cheek. The under-eye area is delicate and the skin is thin, so careful assessment matters more here than anywhere else. If your concern is caused by pigmentation, puffiness or loose skin instead of hollowing, we will suggest a different treatment.

What happens, step by step

Consultation

We talk through your concerns and medical history, examine the area and explain your options, the cost and the risks. You can take time to think it over.

Preparation

On the day we confirm your consent, take photographs for your record, and cleanse the skin.

Treatment

After numbing, a small amount of filler is placed deep under the eye, usually with a blunt cannula through a single entry point on each side.

Afterwards

You leave with written aftercare advice and our number. We check how you have settled at a review if one is needed.

Before

Preparing for your appointment

  • Avoid alcohol for 24 hours before your appointment
  • Tell us about any medication or supplements you take, including blood thinners. Do not stop prescribed medication without speaking to your GP
  • Let us know if you are unwell, have a skin infection or a cold sore in the area, as we may need to rebook
  • Tell us if you are pregnant or breastfeeding, as treatment is not carried out during this time
  • Come with clean skin and no make-up on the area if you can

After

Looking after yourself afterwards

  • Avoid touching or rubbing the area, and leave make-up off for the rest of the day
  • Skip strenuous exercise, alcohol, saunas and steam rooms for 24 to 48 hours
  • Sleep with your head slightly raised on the first night if you have swelling
  • Use a cool compress for swelling, and expect any bruising to fade over a few days
  • Avoid facials, facial massage and dental work for two weeks
  • Call us straight away if you have increasing pain, pale or mottled skin, or any change in vision
  • Swelling under the eyes can take up to two weeks to settle fully

What are dermal fillers made of?

The fillers we use for this treatment are based on hyaluronic acid, a substance that occurs naturally in the skin and holds water.

How long does it last?

Usually 6 to 12 months.

Does it hurt?

Most people describe it as uncomfortable rather than painful. We apply numbing cream beforehand, and most fillers also contain a local anaesthetic.

What are the side effects?

Redness, swelling and bruising at the injection sites are common and usually settle within a few days. Rarer, more serious risks exist with any filler treatment, and we explain them fully at your consultation before you decide.

Can filler be dissolved?

Hyaluronic acid filler can be dissolved with an enzyme called hyaluronidase if needed.

Is everyone suitable?

No. Under-eye bags caused by fat or fluid, or very thin skin, may not improve with filler and can look worse. We will tell you at consultation if another treatment would suit you better.
